This notice mandates us to share information about you unless you tell us not to.
NHS Digital has issued a Data Provision Notice for Planning and Research under Section 259 (1) of the Health & Safety Social Care Act 2021. This mandates us to share your information unless you tell us not to.
This data sharing related to your health data being extracted for secondary use of data purposes e.g. analysis, research, planning etc. It is NOT related to primary, direct care services.
If you DO NOT want to share your data for secondary care purposes, you must complete a Type 1 and Type 2 Opt Out - (National Data Opt Out).
(a) Type 1
The Type 1 opt out relates to information held on your medical record at the Practice. This prevents information held on your medical record at the Practice from being shared for research and planning purposes.

7 Day Service
Urgent and routine appointments with a GP, either same day or in advance are now available in Oldham.
You can now book to see a GP in Oldham in the evening and at weekends. Anyone registered with an Oldham GP will be able to choose from the appointments available across the borough.
NB This is not a walk-in service. These appointments must be booked in advance and may not be with your own GP.
The opening hours are 6.30pm - 8pm Monday to Friday and 10am - 2pm Saturday and Sunday.

Oldham Crisis Response Team
Oldham crisis response team is available to Oldham residents over 18 years of age.
If you are having any difficulties at home you, your relative, friend or carer can refer to the team.
The team can be contacted via Butler Green on 0161 716 2855 between the hours of 8.00am - 8.00pm 7 days a week.

Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.

The Integrated care centre car park has designated parking spaces for disabled patients and a disabled entrance to the building. A wheelchair is usually available on request by phoning the welcome desk at the Integrated Care Centre. Lifts are available to all floors in the building and there are disabled toilets on all levels.
Patient Participation Group (PPG)
We have a PPG which is made up of our patients who give us feedback on a regular basis suggesting improvements in the practice for patient care, eg which day is best for late opening surgery. if you wish to be part of this group who meet on a biannual basis then please enquire at the reception desk for more information.
